A new clinical trial is bringing STM-01 into the spotlight for Duchenne muscular dystrophy. Secretome Therapeutics has launched THRIVE-2, a Phase 2 study exploring a novel approach targeting inflammation and fibrosis in DMD.

Secretome Therapeutics has announced the start of THRIVE-2, a Phase 2 clinical study evaluating its investigational therapy STM-01 in people living with Duchenne muscular dystrophy (DMD). The study is now open for enrollment, with the first participant expected to enroll soon.

What Is THRIVE-2?

THRIVE-2 is designed as a multicenter, multi-dose, open-label Phase 2 clinical trial. Researchers will evaluate both the safety and potential efficacy of STM-01 in people with DMD.

An important feature of the study is its focus on both skeletal muscle and cardiac function. This is particularly relevant in DMD because the disease affects more than the muscles responsible for movement. Progressive damage to the heart can also become an important part of the disease over time.

The announcement describes DMD as a condition involving ongoing muscle injury, chronic inflammation, and fibrosis. These processes can contribute to the progressive loss of skeletal and cardiac muscle function.

What Is STM-01?

STM-01 is an investigational biologic therapy derived from neonatal cardiac progenitor cells (nCPCs).

Rather than being designed specifically to replace the missing dystrophin protein, STM-01 is being developed around a different therapeutic approach. According to Secretome Therapeutics, the therapy is intended to use the regenerative activity of factors released by these cells, known as the Secretome.

The company says these naturally occurring bioactive factors have anti-inflammatory and anti-fibrotic properties. The goal is to influence biological pathways involved in chronic inflammation, fibrosis, and tissue repair.

In simple terms, the approach is aimed at addressing some of the biological processes that contribute to ongoing muscle and tissue damage in DMD rather than directly correcting the underlying genetic mutation.

Why Are Inflammation and Fibrosis Important?

DMD causes repeated muscle injury. Over time, damaged muscle tissue can be replaced by fat and fibrotic tissue. Chronic inflammation can also contribute to this process.

According to investigators involved in THRIVE-2, targeting inflammation and fibrosis could potentially address important mechanisms involved in tissue injury and remodeling.

However, STM-01 remains an investigational therapy. Preclinical findings cannot establish whether the treatment will be safe or effective in people with DMD. The purpose of Phase 2 clinical research is to gather human clinical data that can help answer those questions.

What Have Preclinical Studies Shown?

The announcement reports that, in head-to-head preclinical studies, STM-01 produced greater improvements in cardiac function and greater reductions in cardiac fibrosis than several other stem cell approaches. These findings were cited by an investigator participating in THRIVE-2.

These results are encouraging areas for further investigation, but they should not be interpreted as evidence that STM-01 will provide the same benefits in patients. Clinical trials are necessary to determine whether laboratory and preclinical findings translate into meaningful outcomes for people.

What Happens Next?

THRIVE-2 is expected to provide additional information about STM-01 in people with DMD, including its safety and potential effects on skeletal and cardiac muscle.

The company stated that information about participating clinical sites and eligibility criteria will be made available through ClinicalTrials.gov as sites begin enrolling participants.

ClinicalTrials.gov is maintained by the U.S. National Library of Medicine and provides information about registered clinical studies and their results.

For the Duchenne community, THRIVE-2 represents another clinical research effort exploring a treatment strategy focused on inflammation, fibrosis, and tissue repair. The coming clinical data will be important for understanding whether STM-01 can translate its preclinical potential into meaningful benefits for people living with DMD.
